We believe in personalized care plans, so there is not a one-size-fits-all plan. Nursing & Aide visits are an important part of each individualized care plan. We believe these visits should be tailored to your needs.

  • Nursing

Skilled nursing is the core component of hospice care and is managed by nurses who work continuously with the interdisciplinary team to ensure patients receive the best care possible. A nurse’s duties include monitoring changes in a patient’s condition, reviewing medication, assisting and educating caregivers, and making sure all other needs are met. Nurses work with the hospice medical director to ensure that pain and other symptoms are well managed and that medications are effective.

  • Aides

Working hand in hand with nurses, our Hospice Care Aides (certified nursing assistants) are specifically trained to work with individuals in hospice care. Under the direction of your nurse, the Aide helps with activities of daily living, personal cares, light homemaking, companionship, and more. Aides are involved in care to ensure each patient’s comfort and safety in the home and to meet non-medical needs.
